Managing your life and diabetes can be challenging. The certified diabetes educators at Lake Hospital System can help you control your diabetes with a wide range of support services.

We offer services for individuals 16 years of age and older with Type 1 and Type 2 diabetes.

  • A physician's referral is required.
  • Most major insurance companies and Medicare cover diabetes self-management training.

  • Scholarships are available.
