Individualized Care

Working with your physician and based on your symptoms, the CHF Clinic will develop an individualized treatment plan that's right for you. At each visit, our nurse practitioner will monitor and assess your medications, blood pressure, weight, and breathing for any changes in your condition. If there is a change in your condition, our nurse practitioner will be able to make any necessary adjustments to your medication or treatment plan.

Ongoing Support and Education

During your visit, our nurse practitioner can help you understand your disease by explaining the symptoms and physiological effects of your condition, offer helpful nutrition counseling, and answer any questions. The clinic also provides access to a valuable support network including a dietitian, social workers, and physical and respiratory therapists, and can coordinate physician and therapy visits, too.
